1. Fill out this form: This helps us submit a request for your LOA to your HMO.
2. We'll handle it from here: We will process and submit the LOA request to your HMO.
3. Looking for the LOA? We will send you a notification email once your LOA request is approved. If disapproved, we will also send a notification with the reason for the disapproval.
4. Claim your LOA On the day of your visit, present your approval email or reference number at the designated HMO office, along with your HMO card and a valid ID, to claim your LOA.
5. Remember: Your LOA is only valid within a certain number of days. Please check with your respective HMO Provider or HR representative for clarification.
