The Patient Navigator helps the patients navigate the healthcare system, guides them throughout the different phases of their journey and ensures continuity of care. This is done through relationship building rooted in compassion and aligned with the hospital’s human-centric program “Care beyond Cure.”
The Patient Navigator serves as a bridge and voice to help the patients overcome barriers and complexities in the Healthcare system.
Job Overview
- Assists patients and their families with their inquiries and takes the initiative in addressing their concerns
- Acts as liaison for patients and doctors
- Interacts in a collaborative way with all units, physicians and staff of the organization at all time, expediting patient’s needs
- Acts as a coordinator to help move through the complexities of the hospital like billing, admission, etc
- Implements doctor’s groundwork for treatment like scheduling other doctors, diagnostic procedures etc.
- Helps in the access of prescription and medications
- Documents doctor’s meetings and tracks procedures, medications and changes
- Keeps track of patient’s appointments and missed ones whether for consultation or diagnostics and reminding them to follow through
- Records patient’s issues and status and produces accurate summary of various patient and guest’s feedback
- Takes complete and accurate messages, ensuring that the concerned party be informed immediately
- Provides education to extended family members for continuum of care
- Maintains confidentiality of patient at all times
- Develops a trusting and personal relationship by knowing the patients and their families
- Provides emotional support, being aware of family members’ and patient’s needs and feelings
- Communicates effectively, and provides outstanding patient service
- Ensures availability to patients (except on official leave) via phone and face to face depending on the situation
- Ensures safekeeping of all work-related records and documents
Qualifications
- Registered Nurse
- Oncology experience, an advantage
- With experience in Customer Care, Healthcare or Hospitality Care
- Well-groomed with a delightful personality
- Highly dependable, Patient and a Multi tasker
- Self-motivated with a high emotional intelligence
- Willing to be based in Arca South, Taguig City
