The Patient Navigator helps the patients navigate the healthcare system, guides them throughout the different phases of their journey and ensures continuity of care. This is done through relationship building rooted in compassion and aligned with the hospital’s human-centric program “Care beyond Cure.”
The Patient Navigator serves as a bridge and voice to help the patients overcome barriers and complexities in the Healthcare system.
Referrals for Patient Navigation
- Coordinates with the attending physician for patients referred for navigation services
- Patient’s medical history
- Treatment plan
- Other medical orders
- Coordinates with the patients referred for navigation services and gathers initial patient information through interview, gathering of medical records, providing health education and advice, and encoding into the EMR via Arcus Air
Referrals for Pain Management
- Receives patient referrals for Pain Management (patient assessment, medical history, documentation)
- Coordinates with Pain Specialist for patients referred for pain management services
- Carry out medical interventions as ordered by the pain specialist
- Monitor patients referred to pain management services
- Updates the physician/s of the patients’ condition, compliance to treatment plan, and outcomes of intervention
Patient Coordination
- Provides updates to the physician/s of the patient’s condition and compliance to the treatment plan
- Serves as the patient advocate and resource person – addresses patient’s immediate medical concerns prior, during, and after treatment
- Coordinates the patient’s treatment plan to the different service units of the hospital (laboratory, imaging, treatments, tests, etc.)
- Develops a trusting and personal relationship by knowing the patients and their families
- Provides emotional support, being aware of family members’ and patient’s needs and feelings
Multidisciplinary Team Conferences (MDTC)
- Coordinates the schedule of the Multidisciplinary Team Conferences (MDTC)
- Identifies common availability of all doctors within the team, as well as the patient
- Gathers all pertinent medical records of the patient and sends to the doctors within the team
- Serves as the patient advocate and prepares for MDTC through patient assessment, interviews, and addressing concerns and inquiries of the patients prior to MDTC
- Attends the MDTC and takes minutes of the meeting
- Accomplishes MDTC reports and treatment plan and routes them for approval by the members of the patient’s healthcare team
Administrative Support
- Handles custody and safekeeping of medical supplies needed
- Ensures appropriate use of medical/office supplies
- Prepares regular requisition of supplies according to established inventory scales
- Performs month-end inventory of medical supplies along with the necessary documentation
- Implements and ensure strict compliance to all applicable hospital procedures
- Maintain upkeep of the Clinic, including medical tools and instruments
- Submits timely reports needed for the Monthly Analysis of Performance and other ad-hoc reports as required
- Assist in preparing cost analysis, as requested (Clinic Operations)
- Maintains cleanliness and orderliness in the work area (5S, etc)
- Answer emails, phone calls, and messages for patient’s scheduling and inquiries
- Other duties that may be assigned or other ad hoc tasks and projects that may be assigned
Qualifications
- Registered Nurse with bedside experience, preferably in a tertiary hospital
- Oncology experience, an advantage
- With experience in Customer Care, Healthcare or Hospitality Care
- Well-groomed with a delightful personality
- Highly dependable, patient and a multi-tasker
- Self-motivated with a high emotional intelligence
- Willing to be based in Arca South, Taguig City
