Unlocking the Benefits
of Your HMN Care Card
- Get Your Very Own Carecard for only Php999. Available at all Healthway Medical Network facilities and FEU-NRMF Medical Center.
- Your HMN Care Card is valid for one year from the purchase date. To enjoy the benefits, simply register and activate the card. You’ll find a QR code for registration in your welcome kit.
- Benefits are exclusive to the registered owner. Please present a valid ID when using your HMN Care Card at any Healthway Medical Network facility and FEU-NRMF Medical Center.
- HMN Care Card discounts cannot be combined with other offers, including Senior Citizen and PWD discounts. You’ll always receive the highest available discounts.
- Coverage of Care Card Members extends up to 80 years old. However, Personal Accident Insurance in only valid until age 65.
- Ensure that you present your physical Care Card to enjoy the card benefits. Alternatively, a photo of the card showing both the front and back will also be accepted. Please note that benefits cannot be granted without either the physical card or the complete photo.
- With your Care Card, you'll automatically be issued a Generika Drugstore Generikard, providing you with loyalty benefits and discounts. Simply present your Care Card at the time of your first generic medicine purchase to get your Generikard.

- What are the accepted modes of payment?
- In-Facility Purchase: We accept Cash, Credit Card, Debit Card, and GCash.
- Corporate Accounts: Please coordinate with your account officer for payment options.
- Is the Care Card reloadable?
- No, the Care Card is a prepaid card with a fixed value. It is not reloadable or convertible to cash.
- Can the Care Card be renewed?
- No, the Care Card cannot be renewed
- Can I purchase multiple Care Cards for personal use?
- Yes, you may purchase multiple Care Cards for your own use.
- Can I purchase a Care Card as a gift for someone?
- Yes, you can buy a Care Card as a gift. However, the recipient must register and activate the card. The services included in the Care Card are intended for use by the registered cardholder only. It is prohibited to use the Care Card for purchases intended for someone else.
- One registered card per transaction is allowed.
- Can I use my Care Card immediately after purchasing it?
- The Care Card must be registered and activated before you can avail of any services.
- Can I still use my old "white" Care Card?
- Yes, you can continue using it as long as it's active and registered.
- Can I purchase the new Care Card even if I still have an active old Care Card?
- Yes, you're welcome to have both cards as long as they remain active and registered.
- I still have the old Care Card that is not yet active and registered. What should I do with it?
- You can register you old Care Card until November 30, 2024, so you can still enjoy its benefits!
- Can I avail the benefits of the new Care Card if I only have the old Care Card?
- You can still register your old Care Card until November 30, 2024, allowing you to enjoy its benefits. Please keep in mind that after that date, we won't be able to accept registrations for old Care Cards.

- Who is eligible for the Group Personal Accident Program?
- All Registered Care Card Holder. Ages 1 to 65 years old.
- Am I covered even after work hours or while outside the country?
- Coverage applies WORLDWIDE, 24 HRS/day; 365 days a year, On & Off the job.
- Are animal bites (including mosquito bites) covered in the program?
- Yes. Accidental Death benefit coverage due to the Animal bite such as but not limited to Dengue and Rabies will be covered.
- Can I designate my common-law partner as a beneficiary?
- Yes. Beneficiary designation letter to be submitted indicating desired beneficiary plus supporting documents (e.g Certificate of Cohabitation, Cenomar).
- Will I be covered should an accident occur while riding the company-provided shuttle service?
- Yes! Accidental Death benefit coverage shall be covered for any accident cases subject to policy provisions.
- Do we cover employees riding a motorcycle or bicycle going to work who happens to meet into an accident?
- Yes, it is covered if the employee is not under the influence of alcohol or drug addiction. If the employee is riding a motorcycle, he or she must have a valid driver’s license and must wear the proper gear required. So, for as long as no illegal acts or violations of law, it will be covered.
If Your Care Card is Lost
- Contact HMN Customer Service at 0917 466 2273 or care@healthway.com.ph immediately to report the lost card and request deactivation.
- Provide the following details:
- Full Name
- Birthdate
- (Optional) Card Number
- Last transaction made at clinics or hospitals
- Location where the card was purchased
- Processing Fee: A PHP 200 fee will be charged for card replacement.
- Replacement Timeline: The replacement card will be issued within 7 business days.
- Collection: A representative from the clinic or hospital will notify you when the replacement card is ready.
- Identification: Present one government-issued ID when collecting your new card.
Note: If you’ve used your free flu shots or APE with your lost card, you can’t use them again with the new card. However, you can still access discounts and unlimited consultations.

- Imaging Services: Diagnostic procedures that use various technologies to create visual representations of the inside of the body. Xray, Ultrasound- general and OB, Mri, Ctscan, Heart station - ecg, 2d echo
- Flu Vaccine: An immunization shot designed to protect against influenza (flu) viruses. It helps prevent the flu and reduces the severity of symptoms if contracted. Subject to vaccine availability. Contact HMNN Vaxhub for appointment.
- Annual Physical Exam: A comprehensive health check-up performed once a year to assess overall health, detect potential issues early, and update vaccinations. It typically includes a physical examination, health history review, and may involve routine tests and screenings. (Basic 5 – Urinalysis, Fecalysis, Complete Blood Count, X-ray, Full Medical Exam)
- Generics Medicine: Medications that are identical in formulation, dosage, and effectiveness to their brand-name counterparts but are sold under their chemical names and are usually less expensive.
- Hospital Emergency Room: A medical facility or department within a hospital that provides urgent and emergency care for acute injuries, illnesses, or conditions requiring immediate attention. Discounts apply only to the use of the ER.
- Hospital Room: A designated area within a hospital where patients stay during their treatment or recovery. Rooms vary in type and amenities, ranging from private rooms to shared accommodations.
- Group Accident Insurance: An insurance policy that provides coverage for injuries resulting from accidents. This type of insurance typically covers medical expenses, accidental death, and dismemberment for a group of people, such as employees in a workplace or members of an organization.
- Teleconsultation: allows you to receive healthcare remotely through phone calls, video calls, or online messaging. With your Care Card, you can enjoy unlimited free phone consultations; please note that video consultations will require an additional payment.
